State and federal processing typically returns a result within a few business days, and posting to a board or to the Clearinghouse follows from there. We take it at our West Palm Beach office in the same appointment as your fingerprints, against the neutral background the specification requires. You don't need to bring or arrange a photograph.
Ordinary clothing is fine. Hats and sunglasses come off, as with any identity photograph. There's no dress code and nothing to prepare.
Generally five years, provided your employment status is kept current in the system by an associated provider. If you leave the field and the record goes dormant, you may need rescreening on return.
